Review of Death Investigation (2012-02-14)

Dear Colleagues,

Many of you may be aware that the Ministry of Community Safety and Correctional Services are conducting a review of the system of death investigation in Ontario. KPMG LLP has been engaged to provide a comprehensive and objective report of analysis and recommendations.

The Ontario Association of Pathologists has been identified as one stakeholder in this review. This week, we participated in a preliminary interview with the KPMG consultants. Some of you may have already participated or will be participating in this review as well. However, if not, we invite you to complete the attached questionnaire. All responses will be confidential and forwarded as aggregate data to KPMG.

The OAP does not have a formal position about a preferred model of death investigation in Ontario. However, the OAP board is supportive of this review that will make recommendations based on the data/evidence collected. We welcome the opportunity to provide input from all OAP members.

Unfortunately the timelines of the review are very short. Please submit your responses by Feb. 16, 2012.

To complete the survey, please right click and save to your desktop or other location on your computer. After completing the survey, email or fax to Dr. Chorneyko.  The file is in Microsoft Word format. Please send responses electronically or by fax to: Kathy Chorneyko Email: choka@bchsys.org or Fax: 519 752 7809

Sincerely,

Kathy Chorneyko (President)
David Shum (Vice-President)

Globe and Mail Pathology Article (2012-01-03)

Dear OAP Members,

On Tuesday Jan.3, 2012 an article was published in the Globe and Mail by Andre Picard discussing errors in pathology and lack of quality assurance ("What we should learn from our pathology problems").

A very good response was published from Dr. David Driman ("Changed Pathology" 2012-01-05 Letters to the Editor).

A response was also submitted from Dr. Meg McLachlin and myself on behalf of Path2Quality in order to provide balance and information about substantial work which has been done in regards to quality.  This response has not been published to date, but hopefully will be.  For your information, please find it here.

Sincerely,

Kathy Chorneyko
President
